Current Assessments & Improvement Plans
Counties are encouraged to submit annual progress reports on their health improvement plans. These reports help to track progress in achieving improvement goals and strategies. The following reports highlight some of the innovations and partnerships being undertaken across the state. In addition, each county's most recent CHA CHIP and progress report is posted below. The counties welcome questions about their efforts. Contact the county directly for more information.
