Vaccine

While there are two vaccines available in the U.S. to prevent monkeypox (MPV), there is very little supply in Iowa. To best reach people who are at highest risk of MPV, we are working with a few local public health departments and health care providers that already care for people at highest risk for MPV disease to vaccinate their patients. 

Strategy

Providers are using the following strategies to use this limited vaccine:

• Post-exposure prophylaxis (PEP): Provide vaccine to people who know they had an exposure to MPV.
• PEP++: Provide vaccine to people who are at the highest risk for MPV, even if they don't know if they had an exposure to MPV.

Allocation

• Iowa’s allocation of the JYNNEOS vaccine is not sufficient to implement a statewide vaccination strategy which includes a large number of healthcare provider sites. Only select healthcare partners will serve as PEP++ providers in Iowa.
• The amount of vaccine being provided to our state is only enough to vaccinate a few thousand people in Iowa. That means we need to prioritize people at highest risk for severe MPV disease and those most likely to have been recently in contact with someone with MPV.

Eligibility

PEP++ is for people with unconfirmed exposure to the virus but with risk factors that may make them more likely to get monkeypox. Due to the high demand, and current supply shortages, the
following are the current eligibility requirements to receive this vaccine.owans with the highest likelihood of exposure to monkeypox MUST be prioritized to receive the vaccine. Data indicates most, but not all, cases occur among gay, bisexual, and other men who have sex with men.

• PEP++ Eligibility:
  • Gay, bisexual, other men who have sex with men (MSM), or transgender people* and their partners
  • People living with HIV
  • People who report being at a venue in which a suspected, probable, or confirmed case of monkeypox was identified
  • People who report having close contact with someone suspected, probable, or confirmed as having monkeypox.

Vaccine Administration

• To increase the number of people at risk that can be vaccinated, the CDC and FDA recommend adults 18 years of age or older who are at high risk for being exposed to the monkeypox virus be vaccinated with a 0.1 mL dose of the JYNNEOS vaccine that is administered intradermally (between layers of skin) instead of subcutaneously (underneath the skin).
• Administering the vaccine intradermally for adults 18 years of age or older is an important way to increase the number of people who can be vaccinated because of the lower dose used while still offering a high-level of protection as measured using similar methods.
• The CDC does recommend that eligible individuals receive the JYNNEOS vaccine regardless of previous vaccine history.

Efficacy

• When the vaccine is administered intradermally, it has been shown to produce nearly identical levels of antibody protection compared to a higher dose of the same vaccine administered subcutaneously (i.e., intramuscularly).
• Intradermal administration has been effective for other types of vaccine including yellow fever, polio and rabies.
