Best practice for immediate changes to your contact information: update your profile by accessing the online services page. You will need to 'Sign In' to access your profile.
The Board primarily utilizes email for communication with the licensee. It is your responsibility and in your best interest to also keep your email updated.
If you cannot update your profile you can print the below form and mail it to the Board office. Allow 2-3 weeks for processing.
