How to Renew a Tanning Permit

• IDPH will send you an email reminder approximately 45 days in advance of the expiration date of your permit. You may then submit the renewal either electronically or by printing off the form found in the link below, along with the $5 renewal fee.

REMEMBER: Every five years you must provide proof of completing the owner test taken with the county health department with your renewal. The renewal will not be processed unless you have a current test on file.
