Diet Modification Request Form

This two-page (8 1/2 x 11) form is for child care programs who participate in the Child and Adult Care Food Program (CACFP). CACFP is a federally funded program that provides meal reimbursement to child care centers, adult day care centers, emergency shelters, before and afterschool programs, and child development homes. The Diet Modification Request Form is completed for children with special dietary needs. The form was developed by the Iowa Department of Education, Bureau of Nutrition and Health Services (11/2017).

Exposure Control Plan & Sample Sharps Injury Log

The Exposure Control Plan is intended to serve as an employer guide to the OSHA Bloodborne Pathogens standard.  This 10 page (8 1/2 x 11) document has an easy-to-use format for writing an exposure control plan. The Sample Sharps Injury Log is for documenting needle stick or other sharp injuries for complying with OSHA standards.

First Aid Checklist

This updated (April 2022) 1-page (8 1/2 x 11) first aid checklist may be used by child care providers for assessing and monitoring first aid kits.

Good Neighbor Iowa 

Good Neighbor Iowa is a statewide public education campaign to reduce children’s exposure to commonly-used lawn pesticides. It involves school districts, park managers, child care businesses and other community leaders who are demonstrating that it is possible and practical to manage large areas of turf without the use of pesticides (weed killers, insecticides, or fungicides). Good Neighbor Iowa has a Lawn Care and Pest Management Policy available for child care businesses.

Grief and Loss Support Fact Sheet

This fact sheet provides information on supporting children with grief and loss. The Grief and Loss Support Fact Sheet is in two formats, a 8 1/2 x 11 3-page printable color handout and a 4 1/2 x 11 color bookmark (front and back).

Head Lice Brochure

This brochure provides tips and treatment information about head lice as recommended by the Iowa Department of Public Health.  This is a 2-sided (8 1/2 x 11) color brochure.

Infant Daily Report

This 2-sided (8 1/2 x 11) handout provides child care providers with a method for communicating with families about the care of their infant. The document contains a section for parents to report the status of their infant upon arrival at child care. The document has space for the child care provider to report the infant's status and care given.

Tobacco and Nicotine-free Policy Guidelines

The Tobacco Use Prevention and Control Division at the Iowa Department of Public Health (IDPH) has established a Best Practice policy for child care homes and centers. The Tobacco-Free/Nicotine-Free Policy guidelines for use with Preschools and Child Care Centers/Homes model policy meets the Tobacco and Nicotine-free policy requirement for Iowa Quality For Kids (IQ4K™) Level 2-5.
