Industrial Radiography Trainer & Trainee Cards

Process for Applying or Accessing your Account Online
To add trainer to an existing Iowa radiographer card online, you must complete an online service application. Please navigate to IDPH portal page link, go to the page listing your licenses, and click on online services next to your radiographer license.
To request a replacement card online, you must complete an online service application. Please navigate to IDPH portal page link, go to the page listing your licenses, and click on online services next to your radiographer license.
Process for Applying by Paper
Complete the appropriate paper application listed below and mail application and fee to the address described on the application.
