How do I register my new facility?

A new swimming pool, spa, spray pad, or water slide can be registered by completing the Registration Form and submitting the applicable fee(s) to the Swimming Pool and Spa Program.  Please use the form below for a new registration. The fee for a new registration is $35 per pool, spa, spray pad, or water slide.

*PLEASE NOTE:  This form is for a NEW pool, spa, etc. See the next subheading "How do I renew my facility?" for a renewal form. Incorrect forms will be returned.

How do I renew my facility?

The most efficient way to receive your renewal registration is to renew online. To renew now, visit the Regulatory Programs Portal.

Registration renewal notices are emailed out on or around March 1st of each year.  RENEWALS WILL NOT BE ACCEPTED BEFORE MARCH 1ST. The fee for a registration renewal is $35 per pool, spa, spray pad, or water slide. Late fees will start May 1st of each year and are ongoing until the renewal registration is paid.  The late fee is $25 per month for each pool, spa, waterslide, etc that is required to be registered at the facility.  Your registration will be emailed the following day to the email address we have on file.  Please use the form below to renew your registration.

How long is my registration valid?

The registration for a swimming pool or spa is valid May 1st through the following April 30th (i.e. May 1st 2021 - April 30th 2022).  The registration should be renewed annually on or before April 30th.

What if I am late renewing my registration?

Renewal registration fees are delinquent if not received by the department by April 30th of the first business day thereafter.  There is a $25 penalty each month for each pool, spa, spray pad or water slide that is required to be registered.

What if the ownership, name or other registration information for my facility changes?

The new owner should notify IDPH within 30 days of the change of ownership of a swimming pool or spa by completing the record change form.  The department should also be notified in a timely manner of any changes in the facility name or other change in registration by completing the record change form.  The fee for registration changes is $20 per swimming pool or spa.
